The Trinity Health Of New England Medical Group seeks a Nurse Practitioner or Physician Assistant to join the Heart Failure team as a VAD Coordinator.
Join a highly respected, rapidly growing, heart failure interdisciplinary team with three board-certified heart failure attendings, advanced practice providers, and heart failure disease management RNs for the opportunity to help develop and grow a ventricular assist device program. The VAD Coordinator will be involved in the development of the VAD program and the coordination of Heart Failure patients being evaluated for mechanical circulatory support and following VAD placement in collaboration with the clinical and administrative leadership of the Heart Failure Team. This nurse practitioner/physician assistant collaborates with patients, families, peers, physicians and other members of the health care team in developing the health care services for this patient population. Program development includes but is not limited to working with industry and hospital stakeholders to develop patient pathways and protocols as well as staff and community education. Patient care coordination includes but is not limited to patient education, staff education, community education, clinic visits, being available for call coverage, record keeping and evaluation of the medical and nursing regimens in accordance with the hospital and nursing philosophy, policies/procedures, and standards. He/she demonstrates leadership/management skills and a commitment to professional accountability and growth. This position includes responsibilities for clinical, fiscal and administrative management of the VAD patient population.
