Heart Failure Outpatient Advanced Practitioner opportunity in the Hartford and Waterbury region.

The Trinity Health Of New England Medical Group seeks a Nurse Practitioner or Physician Assistant to join the Heart Failure team.

Join a highly respected, population health focused, heart failure interdisciplinary team with three board-certified heart failure attendings, physician assistants, nurse practitioners, case managers and disease management RNs. As a member of the outpatient team, you will be responsible for managing heart failure patients throughout the continuum of care. Daily responsibilities include seeing patients in various Heart Failure Center’s throughout the region for transitional care management, guideline directed medical therapy optimization, same day access, and diuretic infusions. Responsibilities will also include rounding at partnered skilled nursing facilities. The successful candidate should have experience working in cardiology, but highly motivated new graduates with an interest in advancing in a heart failure care setting will also be considered.
