Heart Failure Inpatient Advanced Practitioner opportunity, Hartford, Connecticut.
Trinity Health Of New England seeks a Nurse Practitioner or Physician Assistant to join the Cardiology Heart Failure team at Saint Francis Hospital and Medical Center.
Daily responsibilities include:
- Seeing patients admitted with heart failure in consultation with the Cardiology team
- Performing advanced Heart Failure consultations
- Working with the interdisciplinary team made up of hospitalists, social workers, nurses, Cardiologists
- Working as an extension of the physician in managing patients with Heart Failure
- Skillset includes:
Management of diuretics
Guideline directed medical therapy initiation
Management of cardiogenic shock
- Attending interdisciplinary rounds daily
The successful candidate will:
- Be interested in Heart Failure care
- Have Cardiology/Heart Failure experience (preferred but not required)
- Hold an Advanced Practice Nurse or Physician Assistant CT state license
