The Trinity Health Of New England Medical Group seeks a Nurse Practitioner or Physician Assistant to join the Heart Failure team at Saint Francis Hospital and Medical Center in Hartford, Connecticut.
Join a highly-respected, heart failure focused, interdisciplinary team with two board-certified heart failure attendings, physician assistants, nurse practitioners, cardiology hospitalists, case managers and RNs. As a member of the inpatient team, you will be responsible for managing the care of hospitalized heart failure patients. Daily responsibilities include rounding with attendings, admitting and discharging patients, and participating in interdisciplinary rounds. The Heart Failure program has a strong emphasis on guideline directed therapy and transitional care. The successful candidate should have experience working in cardiology, but highly motivated new graduates with an interest in advancing in a heart failure care setting will also be considered.
