Trinity Health Of New England Medical Group seeks an experienced, licensed Nurse Practitioner to join our Geriatric Care team seeing inpatients at Saint Mary’s Hospital in Waterbury, and providing office-based care in Prospect, Connecticut.

Due to unprecedented growth and an expanded scope of services, our Section of Geriatrics has an exciting full-time opportunity for a skilled APRN to join our dedicated geriatric care team.  In this role, you will work closely with the inpatient care team of board-certified physicians and advanced practitioners to provide dementia care, pain and symptom management, goals of care conversations and psychosocial support for admitted patients and their families. As an integrated hospital, you will be able to work with providers across the full spectrum of on-site specialists to coordinate the best possible care for patients.  You will also join our office practice and provide geriatric consultations and follow-up care.   You will have ample opportunity to maximize your clinical skills within this department while pursuing additional training, educational opportunities, and research. This is a full-time position. We are open to either 32 or 40 hours per week.  We are seeking a collaborative professional who pursues excellence and is driven to provide patient care with respect and caring.
